Medicaid Recertification

Your local Department of Social Services may need information from you to complete your Medicaid recertification. Be sure to complete and return your forms as soon as possible so you don’t have a gap in your Medicaid health coverage.

If you did not get a letter in the mail, we may not have your current mailing address. If your address has changed, contact your local DSS to update it.
If you have an enhanced ePASS account, you can update your address and other contact information online. You may even be able to use ePASS to return your renewal information.

Complete and sign the form. If you get one of these notices, fill it out and return it to your local DSS. If you think you received a letter that was not from your local DSS and it may be a scam, contact your local DSS.

You can return the form by any of the following ways

Online at epass.nc.gov with an enhanced ePASS account.

Mail

Mail the form back to your local DSS office. Find their address here.

Phone

Call your local DSS office. Find their phone number here.

DSS Office

Visit your local DSS office and drop off the form. Find the nearest office to you here.

Fax

Fax the form back to your local DSS office. Find their fax number here.

Email

Submit the information by email. Go to your local DSS website to see if they accept information by email.
